 Our Functional Capacity Assessment for Dual Sensory Loss is designed for individuals with both hearing and vision loss, providing a comprehensive evaluation of your ability to perform daily activities. As with the Functional Capacity Assessments, we assess areas such as self-care, mobility, communication, and social interaction.  A detailed report that supports NDIS funding applications is provided and informs appropriate interventions. Our goal is to enhance your independence and quality of life with appropriate recommendations. 

  Our Supported Independent Living (SIL) Assessments are designed to evaluate your support needs to ensure you can live in a safe and supportive environment. We complete a comprehensive assessment of your living situation, your daily living skills, home safety, cognitive functioning, and physical capabilities. Recommendations are customised to meet your individual needs, and a detailed report that supports NDIS funding applications is provided to ensure that you can pursue your goals, build capacity and achieve safety and independence at home.  

  Individualised Living Options (ILO) are services provided through the NDIS to help you live independently in a safe and supported environment. It is designed to be flexible and includes both NDIS supports, and informal supports in your life.
Our ILO Assessment looks at your individual support needs and how you would want to live in the future. We assess your daily living skills, support preferences, your safety and security, as well as your cognitive, emotional and cultural well-being. A detailed report that supports NDIS funding applications is provided to ensure you have the right supports, empowering you to take control of your living situation and improve quality of life. 

  Our Assistive Technology and Equipment Assessment is designed to enhance your independence and quality of life. We assess your individual needs, abilities, and goals across areas such as mobility, communication, and daily living activities. We'll explore solutions, considering your home and community environments to recommend the most suitable devices and equipment. Through collaborative trials and personalised recommendations, we ensure that the selected assistive technology integrates into your daily routines. Our assessment supports NDIS funding applications and includes set up and training on equipment use.   

 Our Home Modification Assessment is designed to enhance your independence and safety in your living environment. We identify what you need in your home for your daily routines and assess your safety. 

We then determine suitability to develop a recommendation, ranging from minor adjustments to major renovations. The resulting report supports NDIS funding applications and guides the implementation of modifications, ensuring that changes align with your goals and promote your independence at home.
